Where a block stands in the flow it is rendered in, which decides the gaps around it: the first block takes no gap above, the last none below, and a heading or rule counts the block before it.

The position of blocks[ix] among its siblings. Blocks that render nothing (definitions, breaks) are skipped when looking for the block before it.
