Tracked health of the Tantivy index relative to LMDB.

Because Tantivy indexing is best-effort (an indexing failure does not roll back the LMDB write), the index can drift from the store. This struct tracks successes and failures so wm doctor and system.health can report degraded state instead of silently claiming healthy.
