One event in a trace, tagged by t — the same shape a live captured stream would emit.

ToolResult deliberately carries decision/exit_code rather than a generic “output” blob: those are the two forms the blast-radius gate’s verdict can arrive in (warble blast-radius’s JSON decision field, or the CLI’s own exit code convention — 0/10/11), and the scorer only ever needs to read a gate’s verdict off a ToolResult, nothing else about it.

Approval
