deny verdict — the policy refused a well-formed, verified request.

code is the policy’s stable refusal code; reason its optional elaboration. A constructor rather than a hand-assembled struct because this shape now has two producers — the correlated ceremony reply and the status poll’s projection — and hand-assembling it in both is how they drift.
