Fields grouped down the left edge, outermost first.

col_fields: Vec<PivotField>

Fields grouped across the top, outermost first.

value_fields: Vec<PivotValueField>

filter_fields: Vec<PivotFilterField>

Fields restricting which source records take part.

grand_totals_row: bool

Whether a grand-total row is appended below the body.
