Identity of an indexed document: which harness’s session it is.

source disambiguates distinct copies that share a (harness, id) — Claude Code, for one, writes the same session id into more than one project directory when a session is resumed from a different cwd — so both copies can be indexed side by side instead of one silently replacing the other. Callers whose ids are already unique may leave it None.
