Baseline for one Tuff-managed mcpServers.<id> entry (RFC-102 stage b).

MCP config files are shared ground that users hand-edit, so the entry gets the managed-hook treatment: a content hash recorded at registration time, compared on every check/list, never whole-file ownership. The entry’s key is the capability id, so only the file path and hash are stored.
