LegacyUnsigned
No . separator, valid base64, decodes to JSON with “tier” field.
LegacyInvalid
No . separator, not valid base64 or missing “tier” field.
SignedStructural
Has exactly one . with two non-empty base64url segments.
Signature/claims may still be invalid — this is structural only.
Malformed
Has . but structure is wrong (empty segments, multiple dots).
