Why a Node::apply_pos_edits batch could not be applied.

A block-structural op failed.

A recorded change failed to apply.

UnsupportedSpan

A cross-block span whose endpoints aren’t sibling blocks under a common parent (different depths/parents) — unsupported in v1.
