One occurrence, in char offsets into the original haystack — never into the folded copy.

The span covers every source char the match consumed, including ones the fold elided: a query cafe matching a decomposed café reports 5 chars, not 4, so that replacing it cannot leave an orphaned combining accent floating on the replacement.
