Errors returned by strict run-artifact validation.

Default analysis remains permissive and reports these as warnings where possible; strict validation is for callers that want to reject ambiguous artifacts before triage.

DuplicateCompletedRequestId

More than one completed request event used the same request_id.

Stage or queue evidence referenced a request_id with no completed request.
