PermissionGuard — 检查用户具有特定权限

对齐 PHP szoa RBAC AuthService::check()：

权限格式

对齐 PHP access 表，格式：controller/action（如 user/list、user/save）。

支持通配符：user/* 匹配 user/list、user/save 等。

is_super 绕过

is_super=true 绕过所有权限检查（对齐 PHP is_super=1）。
