Host-supplied activation authority for MODEL-INITIATED activation.

ModelConfirmed may be populated ONLY by host policy (explicit server auto-approve or an interactive confirmation mechanism) — never from model-authored JSON arguments. Explicit user-requested activation goes through activate_exact_for_user, a host Rust API that involves no authority value at all (PR #63 ergonomics: a user asking for an exact known tool needs no redundant prompt).

No confirmation authority: model-initiated activation is denied before any grant issuance or set/schema mutation.
