The structural outcome known for a tool-result message.
KnownSuccess deliberately remains the default for messages with no
marker: Claude v1 omits is_error on successful results, and live tool
execution already marks only its error branch. Codex v1 imports opt into
Unknown explicitly because their output records expose no structured
success bit.
