Fold every file’s contribution into one index keyed by bare trait name.

Impls are counted for every scanned file, test code included: a trait with one production impl plus a test mock is the legitimate “trait exists to be mocked” pattern, and the mock counting as a second impl is what suppresses the dominant false positive.
