Why Sessions::complete_step_up failed. As a response:

• StepUpFailure::SubjectMismatch: 403 {"error":"subject_mismatch"}, cookie kept.
• StepUpFailure::Rejected: the layer’s 401 for that rejection (see rejection_response).
• StepUpFailure::Guard: 500 {"error":"internal"}, the error only logged.
