The reference_count field is fully managed by the handler — callers
of allocate_video / modify_video in CameraAvStreamHooks should
ignore it; cross-cluster consumers (WebRTC, PushAV) adjust it via
CameraAvStreamHandler::acquire_video /
CameraAvStreamHandler::release_video.
