Backend configuration entry from a ceremony file.

The provider field identifies the backend by its self-declared name (e.g., "software", "yubikey"). The remaining YAML keys are flattened into extra for backend-specific deserialization.

Each backend crate defines its own config struct and deserializes from extra at startup. This inverts the prior design where the model listed concrete backend variants; backends now declare themselves.
