Expand description

The edge-triggered stall decision, separated from the thread so it can be tested. Feed it each observation; it reports a stall once per episode and re-arms after a fresh beat.

last_beat_ms is the last heartbeat and now_ms the current time, both in Unix milliseconds. Returns the report on the first observation of a stall episode.
