Per-TTL breakdown of cache-write tokens (usage.cache_creation).

Distinguishes 1-hour cache writes (~2x base input token price) from 5-minute writes (~1.25x), which is what makes a mixed-TTL configuration (see [CompletionModel::with_static_prefix_cache_ttl]) observable. Unknown buckets a provider may add later are ignored on deserialization.
