Gossip-derived member health inputs keyed by the same NodeIdentity the
supervisor already scores.
The tracker is deliberately just a transport adapter: it folds
signal-plane liveness and health-input messages into MemberSignals, then
the existing HealthPolicy keeps doing all scoring and grace gating.
