Expand description

One source row in the sources frame. Caller produces these after RrfFuser (#398) + column-policy redaction. urn is the engine entity URN; payload is the redacted JSON the LLM also saw, serialized as a string to keep the SSE JSON flat (the client re-parses if it wants structure).
