Run the server on real stdio. Implements 60-second idle timeout (S1-C9): if no input arrives on stdin for 60 seconds, shuts down cleanly with exit code 0 and logs “ServerShutdown” to stderr. EOF (e.g. /dev/null) causes a brief sleep then retry — the server never exits on EOF alone.

llm is the shared chat provider used to construct AgentRuntimes for new sessions. Pass None only when you only need initialize support (e.g. smoke tests, idle-timeout tests).

Run the server on generic reader/writer (testable). Exits on EOF.
