Invariants maintained by SourceText construction:

• entries are sorted by unique carrier/raw byte offset;
• every offset is a UTF-8 character boundary and points at the carrier scalar U+E000 + (unit - 0xD800);
• unit is one UTF-16 surrogate decoded from a three-byte WTF-8/CESU-8 sequence;
• both the original encoding and carrier occupy exactly three bytes.
