Remove this connection’s presence from a stream during in-page navigation.

A full websocket disconnect is already cascade-cleaned by myko, but moving between streams keeps that socket alive. Addressing the same connection-keyed row as JoinStream prevents ghost viewers and immediately reconciles the old room’s wheel.
