State machine that turns the events: Vec<IngestEvent> array into a flat Vec<RowOutcome> matching the array’s index space. Buffers a whole session substream so merge_insert runs once per substream (three batches: sessions, messages, parts). A validation error on a single event drops that event (one OutcomeStatus::Error outcome) and the substream continues; only Session-level invariants (immutable source_agent / project on re-write) drop the whole substream (spec.md#adapter-integrity-event-ordering).

Writes are batched at flush time. As complete substreams arrive (a new Session event closes out the current one), they accumulate in completed rather than each one calling merge_insert immediately. The caller drains the buffer via Self::flush / Self::finish, at which point one batched 3-parallel-merge-insert covers all pending substreams. This is the load-bearing perf change: per-substream commit overhead dominated the ingest profile (see benches/ingest_bench.rs), and amortizing it across N sessions cuts wall time materially.

Returns the per-row outcomes the event triggered: empty when the event is just buffered, or N entries when a session substream just flushed (success or failure). Err is reserved for catastrophic storage failures that should fail the whole pond_ingest request.

Caller invokes this periodically (every N completed substreams) to keep memory bounded; in adapter-driven sync that happens via the BATCH_SIZE check in ingest_adapter. The current in-flight substream stays buffered - close it explicitly via Self::finish or by feeding the next Session event.
