The body of a subscribe request — serialized as the inner object, since the envelope is added when the frame is sent.

Only fields the server documents are emitted. SUBSCRIPTION_TYPE_* is protobuf-style naming and such servers commonly reject unknown fields outright, so nothing speculative is added here; extra carries only what a caller asked for explicitly.
