A RequestPolicy’s refusal of one outbound request.
§Residual: the message is authored by the policy, not by this phase
The message travels back to the MCP client. It is chosen by the policy implementation, so the toolkit cannot mechanically constrain it — a policy that interpolates the rejected value into its own message creates exactly the value-echo leak every toolkit-authored refusal in this phase avoids (T-128-40). Supply a FIXED string. Do not put a request value, a resolved path, or a credential in it.
