Streaming HTTP response returned to the host dispatcher.

This intentionally returns only the response head plus a byte stream. The caller is responsible for chunking/decoding (UTF-8/base64), SSE parsing, idle timeouts, and delivering StreamChunk outcomes to the extension runtime.
