Cwd-bound service handles the runtime needs to drive replacements.

This is a intentionally narrow view of crate::core::agent_session_services::AgentSessionServices: the runtime only needs cwd, agent_dir, and accumulated diagnostics. The full services object (with ModelRuntime, DefaultResourceLoader) is owned by the app layer; the factory extracts what the runtime needs.
