Build from a parsed document, optionally enriched by PHPStorm metadata for factory-method return type inference.

Like [from_doc_with_meta] but scopes method-body variable collection to the method (or function) containing position. Variables local to other method bodies are excluded so they cannot pollute the map with wrong types at the cursor site. Sets $this when the position is inside an instance method.

Returns the class name for a variable, e.g. get("$obj") → Some("Foo").
