The split_by corollary to GroupRollupMode. FLAT (the wire default)
is the historical behavior - only full-depth split combinations are
emitted as columns. ROLLUP additionally emits grand-total and subtotal
column groups in pre-order (“totals before”). Enum value names are
prefixed because proto enum values share their parent scope with
GroupRollupMode’s.
