Compute the quadric error matrix Q for a vertex.

For each adjacent face, a plane equation [a,b,c,d] (ax+by+cz+d=0) contributes the outer product pp^T to Q. Returns a 4×4 matrix.

Edge collapse cost using combined quadric: v^T (Q1+Q2) v.

The optimal collapse position is taken as the midpoint (v1+v2)/2.

Greedy QEM simplification: collapse edges until target_count faces remain.

This is a simplified greedy approach suitable for small meshes.
