Connect to an MCP server via stdio transport.

Performs the full initialization handshake:

1. Spawn the process
2. Install the default inbound responder
3. Send initialize request
4. Send notifications/initialized

Connect through a pre-built transport (used by tests and by future HTTP/SSE transports). Installs the default inbound responder (ping → empty result, roots/list → empty roots, others → -32601) before the initialize handshake so the client does not deadlock on servers that send requests between responses.
