Atomic, monotonically increasing request ID generator.

IDs have the form "{prefix}-{timestamp_ms}-{counter}", e.g. "oxibonsai-1714000000000-42". The combination of a millisecond timestamp and a per-process counter makes collisions practically impossible across restarts.
