Refresh orchestration service.

This flow:

  • looks up session state by opaque refresh token
  • rejects missing, revoked, or expired sessions
  • reloads tenant-scoped roles for fresh auth material
  • issues a fresh access token
  • rotates the refresh token through SessionStore
