A scheduled macrotask (setTimeout/setInterval/setImmediate). Ordering
is by (delay, seq) — a deterministic virtual clock, never wall time.

setTimeout/setImmediate. A repeating timer is re-armed with a fresh
deadline each time it fires, so it keeps the loop alive indefinitely —
exactly like Node, where setInterval runs until cleared.
refed: boolNode’s ref/unref handle bit. Only a referenced pending timer keeps
the event loop alive; an unref’d one still fires while the loop happens
to be alive for another reason, but never holds it open by itself.
deadline: InstantReal wall-clock deadline (now + delay), used only on the real-clock
path (an open handle or a pending interval). On the pure virtual clock
this is ignored.
