Missing

Skip every phase identity already completed in any prior execution of this session. The default.

Changed

Skip every phase identity whose prior_hash == current_hash (program shape unchanged). Phases with no prior completion fall through; phases with prior completion but a different hash re-run.

All

Run every phase. Prior outcomes are preserved as cardinal history under their original exec_id; the new run writes under the bumped exec_id.
