SRD-76 — single phase-end readout. Reads the structured outcome (status, duration, error list, resume cursor) from the ReadoutContext and renders LOD-appropriate detail. Replaces the legacy phase_outcome readout that always assumed success.

Naming: the readout struct is PhaseOutcomeReadout rather than PhaseOutcome to disambiguate from crate::phase_outcome::PhaseOutcome (the data type). The string registry key is "phase_outcome".

Required: every readout declares what kinds it works against. No default — silent fallback to [Phase] would defeat the safety net the validation layer is built on.
