Schema-specialized column decoder — replaces per-column is_fixed() + ColumnType match.

Constructed once per scan from ColumnType slice. The match on this enum in the hot loop allows the compiler to devirtualize via exhaustive match, eliminating the indirect branch that ColumnType match would require.
