§ts: u64Unix epoch seconds at record time.
surface: StringWhich surface returned the refusal: "cli" or "mcp".
verb: StringThe verb the caller invoked: MCP tool name (memstead_create)
or CLI subcommand (create).
code: StringThe typed refusal code (UNKNOWN_SECTION, HASH_MISMATCH, …).
reason: Option<String>The refusal’s closed engine-owned reason discriminator, for
the codes that compute one (see closed_reason) — absent
otherwise, never an empty string or placeholder. Entries
written before the field existed deserialize as None.
