One derivation-staleness finding (agent-trust plan 12): an explicit edge on a derivation-declared rel-type whose baseline differs from the target’s current hash (stale), or that has no recorded baseline at all (unbaselined). Fresh edges are never reported.

The recorded baseline hash (None for unbaselined edges).

The target’s current content hash (“” for an absent target).
