RefinementModeDeleted
The ingest declares build mode refinement — deleted from the binding
vocabulary. Not migrated: refinement-as-writer is gone, so there
is no v2 shape to carry it into.
MalformedProjectionRef
The ingest’s projection field is not the required "<mem>/<name>".

The ingest references a projection that does not exist — a dangling ref. A typed error, never a silent drop.

A binding/projection references a facet that does not exist in its mem — the fold cannot inline what is not there.

OrphanRecords
After folding every binding, medium/facet records remain that no binding referenced. Removing the trees would silently drop their content — refused instead; the operator deletes or binds them first.
