One reviewer seat’s record across review rounds, so a round does not ask an agent that already failed the seat while another one is answering.
failed is a priority set, not a bound: the per-round tried set in
ask_json_wave starts empty every round, so an agent that answered is
never locked out. A failed id is only asked again once nothing else on the
roster is left (once per round, and the rounds are review_rounds).

last_ok: Option<String>The agent that last answered this seat.
last_fail: Option<FailClass>The class of the seat’s most recent failure; cleared by an answer.
