A single hit returned by StoragePort::vector_navigate.

id is the decoded doc id (FT index prefix stripped + hex-decoded, the same rule as VectorHit). hop_depth == 0 means the hit was a direct KNN seed; >= 1 means it was discovered via graph expansion. Lower final_score = better (consistent with L2 distance ordering).
