Builds a posting list that includes term positions (for phrase queries).

Format:

[num_docs: u32] [flags: u8 = 0x01]
Per doc: [delta: vbyte] [tf: vbyte] [pos_0: vbyte] [pos_delta_1: vbyte] ...

Positions are delta-encoded within each document.
