An LSP adapter backed by the Salsa incremental engine.

Wraps a LspMaxDb and a DashMap that maps each open URI to its SourceFile input handle. On handle_did_change, only the text field is updated; Salsa automatically invalidates the memoised ast_diagnostics result.

A separate documents cache holds the live, tree-sitter-incremental Document (text + Tree) per URI. SourceFile cannot hold the Tree itself (it isn’t salsa::Update), so without this cache there would be nowhere to keep the previous tree between edits — every handle_did_change would have to reparse the whole document from scratch before applying the incremental edit, defeating the point of Tree::edit. ast_diagnostics’s Salsa-level memoization (skip recompute when text is unchanged) is a separate, still-valid optimization layer and is untouched by this cache.

Falls back to a from-scratch parse only if no cached Document exists yet for this URI (e.g. a didChange that raced ahead of didOpen).
