The outcome of a successful transition attempt, including no-ops.

changed distinguishes a genuine state transition from a no-op that returns the current state unchanged — the no-op rows in ADR-142’s table are load-bearing, not an absence of behavior, so callers must be able to tell the two apart without re-deriving it from state/terminal_reason alone.
