Load configuration from file (TOML/YAML/JSON) and environment overrides.
Environment variables prefixed with ISSUERD_ are merged on top of
the file values. If path is None, only defaults + environment are used.
Whether server-set browser cookies carry the Secure attribute.
Derived from the configured issuer_url — never from the request:
true exactly when the public scheme is https. Every TLS deployment
(direct, or behind a TLS-terminating proxy where issuer_url keeps
the public https scheme) gets Secure on all authentication
cookies — SSO session, remember-me, flow correlation, and the logout
clears — while plain-HTTP development rigs keep working.
