Append-only ledger of scaffold re-seeds triggered by active-mass-floor
breaches. Non-fatal by construction: the ledger only records; it never
holds a terminal/abort state. The wiring agent threads one ledger through
the joint fit and queries ReseedLedger::reseed_count for diagnostics.
